  • 3+ chronic conditions or doctors

  • 2+ ER visits or hospitalizations in last 6 mo

  • No doctor coordinating the full picture

  • New trouble with words or instructions

Things They Might Say:

  • “I don’t know who’s in charge.”

  • “They told me to follow up, but with who?”

  • “I just hope the doctor’s right.”

  • “They rushed me out.”

  • “I don’t know why I’m on so many pills.”
